Requirements

All coaches are required to complete the following:

• Complete the Online Volunteer Coaches Interest Form

or

• Print, complete and return the appropriate volunteer coaches’ interest form:
• Heights Youth Baseball (pdf.)
• Heights Girls’ Softball (pdf.)
• Heights Recreation Youth Basketball (pdf.)
• Coaching Background Screening Release Form (pdf.)
• Complete the online CDC Concussion Training Course
